EY: Health visiting experiences: Survey – Please share with families

Hammersmith & Fulham Council’s Public Health Team are reviewing the Health Visiting Service to ensure it meets the needs of local babies, children, and their parents and carers.

This short survey should only take about 10 to 15 minutes to complete. Participating in the survey gives families the opportunity to win a £50 high street voucher. All responses will be treated as strictly anonymous and confidential and will only be used for the purposes of this consultation.
