The focus of this session will be completion of the H&F Transfer Form. The session will be held on Tuesday, 4 June, 1 to 2 pm.
It is being run by Andrea Boswell, a specialist teacher from the INSPIRE team. This session will be an opportunity to view the forms used for transitions where you can ask questions about content and what level of information is needed in order to support a smoother transition for both the child and the new setting or school.
This is now a requirement set by London Borough of Hammersmith and Fulham for ALL Early years settings to complete and for all new settings/ schools to read and implement any targets/interventions and make reasonable adjustments to their setting. It will also allow for the possibility of necessary funding to be applied for.
