Transition form: children moving to school in September 2025

Please complete the ‘Early Years Summary Transfer Form’ for children moving to reception in September. This will enable the school to plan for the child’s needs. 

There is one form to complete per child. If the child has SEND there is the extra section on the form to complete. If there is No SEND you will be directed to the next section.

Who to submit the document to:       Send the document directly to the school

When to send the document:              During transitions week which is Monday 2 to Friday 6 June 2025

Support with completing the form:    For support, email [email protected] from the INSPIRE team

The expectation is to complete this for all children but especially important for children with SEND.

This form will be sent directly to settings.
