Transition forms: children moving to school in September 2025
Please complete the universal transfer document to support children transitioning to reception in September. This will enable the school to plan for the child’s needs. In addition, the SEND transfer form should be completed and sent for children with SEND.

During transitions week which is Monday 2 to Friday 6 June 2025.
Support with completing the forms:
For support, email [email protected] from the INSPIRE team
The expectation is to complete this for all children but especially important for children with SEND.
These forms will also be sent directly to settings.
